Purpose of This Agreement
Bros Squad Sport (“BSS”) facilitates registration, marketing, and communication for soccer clinics and camps (“Events”) operated by independent coaches, trainers, or partner organizations (“Host Providers”). While BSS may provide on-site support, coordination, or limited coaching assistance, the primary instruction, supervision, and control of all soccer activities are the responsibility of the Host Provider.
By signing this form, you acknowledge the nature of this relationship and agree to release liability for both Bros Squad Sport and the Host Providers.
1. Assumption of Risk
I understand that participation in soccer involves physical activity and inherent risks — including, but not limited to, slips, collisions, sprains, fractures, weather exposure, and other potential injuries. I voluntarily assume all risks associated with participation, whether known or unknown, even if arising from the negligence of BSS, the Host Provider, or their respective staff.
2. Release and Waiver of Liability
In consideration of being allowed to participate in the Event, I, on behalf of myself and/or my minor child, hereby release, discharge, and hold harmless:
• Bros Squad Sport, LLC, its owners, officers, employees, agents, contractors, and volunteers; and
• The Host Provider, including its coaches, trainers, staff, and facility partners, from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, or property damage that may occur during, before, or after participation in the Event, whether caused by negligence or otherwise.
This release applies to all claims known or unknown, foreseeable or unforeseeable, arising from participation or attendance.
3. Independent Event Providers
I understand and acknowledge that many Events are hosted by independent coaches or organizations not controlled by Bros Squad Sport. I agree that BSS is not responsible for the operations, facilities, or safety practices of these independent hosts, nor for the actions or omissions of their employees or agents.
4. Medical Authorization
I authorize Bros Squad Sport and/or the Host Provider to obtain emergency medical treatment deemed necessary for the participant’s health or safety. I accept full responsibility for any medical costs incurred.
5. Health Certification
I certify that the participant is physically fit and able to participate safely in soccer activities. I have disclosed any relevant medical conditions, allergies, or medications to BSS or the Host Provider.
6. Media Release
I grant permission to Bros Squad Sport and its partner coaches or organizations to use photographs, video, or other recordings of the participant for promotional or marketing purposes, without compensation or restriction.
7. Code of Conduct
I agree that the participant will follow all rules, instructions, and directions from coaches, staff, and officials. Bros Squad Sport and the Host Provider reserve the right to remove any participant for unsafe or disruptive behavior without refund.
8. Acknowledgment and Acceptance
I have carefully read this waiver and fully understand its contents. I understand that by signing, I am waiving certain legal rights, including the right to sue Bros Squad Sport or the Host Provider for ordinary negligence. I sign voluntarily and intend this release to be binding on myself, my heirs, and assigns.
